The long-term care program is intended for people with type 2 diabetes to provide them with optimal and continuous long-term care.
Regular visits to the doctor (ideally every 3 months) and appropriate documentation enable structured and more intensive care – the therapy is adapted to your needs and symptoms.
Thanks to the medical documentation, you avoid unnecessary redundant examinations, but the important medical checks are kept in view: regular eye checks, foot examinations and HbA1c.
The so-called long-term glucose (HbA1c) is best determined every 3 months to keep a good eye on your diabetes. If you don’t want to have your blood taken every time (and your possible concomitant illnesses allow it), we are now offering all participants in the Therapy Active program the opportunity to have their HbA1c determined free of charge from a finger prick. The result is available directly after approx. 10 minutes and we can discuss it immediately.
We determine the intervals for the "big" blood tests individually with you.
You can take part in training courses on healthy eating, adequate exercise and independent blood glucose measurements, allowing you to influence the success of your treatment by yourself.
Long-term complications of diabetes such as foot amputations, blindness, heart attacks and strokes can be reduced and possibly prevented.
Together we will work out therapy goals and steps that can influence your diabetes.
Requirements for participation in "Therapy Active"
- Your diabetes was clearly diagnosed.
- You are willing to actively participate in the program.
- You sign the declaration of participation and consent that your doctor will have ready for you.
